When I can regularly ejaculate due to sex or otherwise I will typically edge for 20-30 mins a day if I can find the time. If I can't guarantee that I won't even attempt to edge.
I have also experienced the 'initial erection problem' and have begun intercourse with a 70% erection at times but find that it quickly subsides when I get into it. I have largely attributed this to my PE routine as I've only experienced it on training days. Also, like you, I have found that when I haven't ejaculated in several days and have been regularly edging, I have a short refractory period after my first orgasm.
Personally, I have never had much success in trying to hit the PONR and kegeling through it, I haven't been practicing long but all I've found is that this just causes me to feel the wave of orgasm for several seconds before feeling a 'low-enjoyment' release. Maybe I need to practice more. When I edge I don't try to get 'close' to the PONR, I usually stop when I know the PONR will be withing a few more strokes.
Also, your term of 'death grip' makes me worried about how hard you're gripping your shaft. I grip firmly for Uli's but wouldn't call it a death grip.
