Welcome yg99! This forum is full of a lot of knowledgeable people. While I can't personally help you other than saying "Hi!", I would recommend seeing a urologist maybe? Keep your chin up!

Instead, I feel that perineum area beginning to swell internally and involuntarily flex as i become more aroused. I also feel orgasm coming much quicker than it should, presumably due to the internally flexing muscles.

The end result is a crappy 70% erection with a somewhat deflated head and severely reduced sensation. I feel like i am going to cum after 1 minute of masturbation despite the numbness, sometimes even when still flacid. . Even when i get "fully" hard, my penis looks and feels smaller & lighter than it used to. It feels like there is something internally preventing the blood from entering my penis like it should, and when i contract my kegel muscles i feel a urethral pain deep inside. And when i do cum, its almost as if someone pinched off a tube inside and blocked most of the semen from escaping. (DHT is involved in proper dispersal, ejac and orgasm) Even when i am otherwise relaxed my pelvic floor area feels tingly and tense.

To me it seems like this is either some sort of prostate infection, or a pelvic floor imbalance. I should also note 3 key things:

      1) i was on propecia for 4-5 years on and off before the problems began, and they seemed to start a few months after ending finesteride for good
      2) i went "no fap" for a month 2 years ago, and these problems appeared to get worse after finishing this 30 day abstinence
      3) I have pretty severe lordosis (excess spinal curvature), which i hear can contribute to erection problems

based off this information, what would you guys recommend i do to start fixing this? I have avoided all sexual contact for years because of this and i'm pretty sure its made me depressed. but i've had enough of this shit and i'll do anything just to be normal again.

Sounds like you are suffering from the DHT overinhibition - and probably estrogen dominance due to fina / propecia usage. DHT protects the prostate against excess estrogen, and so by using a drug you thought would help (it's largely a scam) - you in essence, probably made it worse and are now suffering from estrogen induced changes to spinal plasticity and maybe cartilage alterations.

I wouldn't be surprised if you have huge lipid / cholesterol changes too...

Also you say the head is "deflated" ...this is due to insufficient blood flow and presumably force as well, this normally is maintained by potent androgens and by locally produced nitric oxide..which is probably being downsized by an adrenaline issue - which explains the pelvic floor tension .

You may also have some leftover issues from the inflammation and infection, if indeed there was one for sure.

Check hormones, estrogen, prolactin..these may very well be elevated from propecia use...get extensive blood work..until then, continue with no fap, try to workout and eat a clean diet .....

Extreme fatigue, no motivation, muscular pain, no stamina during exercise, ect. Would a urologist be able to do any of those blood tests? I'd rather consolidate my visits if possible
Endocrinologist, but you might wanna ask the uro anyway. Extreme fatigue and muscular pain..no motivation..these are all low DHT symptoms.

I'd be willing to bet you are having short term , long term or directional/spatial memory problems...as well as either brain fog or anxiety...all of which would be low DHT / high estro as well!

This all sounds like classic finasteride symptoms. I had the exact same symptoms as you and it was a result of finasteride. Flacid, low sensation but still having PE for some reason. I have %100 beat that now. I get hard very easily and I never worry about PE.

Firstly I agree with everything Corageon told you.

Secondly, here are some initial thoughts that will guide you along the way to getting better.

          1. This is primarily a hormonal issue as far as I can tell. The most effective actions I took were all about how to increase my testosterone levels and decrease my estrogen (aka estradiol) levels naturally. This may have had flow on effects, for example, higher testosterone tends to lead to higher serum DHT and changes to thyroid function and body mass and all manner of things so it's hard to pinpoint the causal process that actually made me better. However, despite this, I found it most helpful to use total testosterone and estradiol as the most basic hormonal indicators because once you get those in order naturally, everything else tends to fall into line.

          2. You should focus on "natural" ways to increase testosterone and lower estradiol. I'm not saying this because of the very annoying, very hippie notion that "natural" is always better. I'm saying this because pharmaceutical approaches such as testosterone therapy (where testosterone is administered straight to the blood stream) tend to only fix one part of the problem at a time.

Testosterone therapy will increase your blood levels of testosterone, but it won't increase all the hormones that precede testosterone, and it won't activate the areas of the brain responsible for regulating androgen production and libido and so on.

In contrast, something like say, eating a diet that promotes testosterone production, will address all these factors at once. Diet is also way more powerful than you might think in this regard because it actually produces systemic changes to your entire body. Food is medicine if you do it consistently.

          3. The PE clears up when your testosterone is high.

I don't know why and I need to find more studies on this but I actually found that the higher my total T (and the higher the general level of sexual arousal I had which is associated with high T) the less the PE was an issue.

The good news is I think that "PFS" or "post-finasteride syndrome" is actually way more possible to overcome than people say it is. The problem (in my opinion) is that most people have trouble following through on big lifestyle changes that will actually cure them, so they keep expecting a pharmaceutical drug or hormone replacement therapy to be the magic bullet that makes them normal again. That's not a realistic expectation. Drugs can help but lifestyle changes including diet, exercise and supplementation are more potent because they cause systemic changes rather than smaller isolated changes to your body.

I hope something I said was of some use.
I beat erectile dysfunction with diet and supplements. PM for info!

            Yes exactly.^
Likewise, high estro levels can lead to P.E as well., if not E.D...

Low estro on the other hand produces delayed ejac instead...but more stamina assuming your dht levels aren't crashed...in that case, low estro and low DHT will likely result in severe..almost untreatable E.D
At the same time..I've heard of high dht low e2 individuals who also have issues..but mostly not in ejac but in sensitivity in the first place...however...out of the two..low dht and high estro is definitely way worse.
Will inflame your heart and other organs as well!
Estro can also bloat up the intestines and liver, no good!

I had similar problems after quitting finasteride. Request a full hormonal panel. You very likely could have elevated prolactin. One thing that helped me was USPlabs PowerFULL, but see a Dr. first to get your hormone levels checked.
