From what I've read in various studies, hypogonadism (reduced testicular function/reduced circulating androgens) causes penile atrophy.
So reducing androgens can shrink the penis, restoring androgens can restore penis size but supplementing androgens to supraphysiological levels does not seem to result in significant growth above normal size. There are pieces of the puzzle that need to be found and implemented such as growth factors involved in the healing process and androgen receptor density and sensitivity.
I've read of cases of pituitary tumors in adult women causing increases in gonadotropic releasing hormones resulting in significant clitoral growth. Female bodybuilders usually develope huge clitoral development due to androgen usesage. The clitoris is very similar in structure to the penis but appears to be much more sensitive to androgen stimulation. So it appears in adult women that supraphysiological androgen levels will stimulate genital growth, overriding possible genetic control without exercising the clitoris, although some women do pump to achieve greater size.
