From what I gather from other people, the BPFSL tends to be about 0.5" up to even more than an inch bigger than the BPEL. Any ideas on why this is so? You would think that if you could stretch your penis that far, that you should be able to fill it up that far, right? Does this mean the smooth muscle is weak? Does it mean your blood flow is weak?

I'm guessing there'd be a few factors at play.
When you get an erection you are filling the corpora cavernosa chambers to capacity ... but the penis and it's structures are held in position by ligaments. A manual stretch will be pulling and stretching not only the flaccid penis, but also those ligaments. One way to consider that (and you should approach this carefully) but it is possible to do an erect stretch ... obtain a rock hard erection, and still be able to stretch that erection further by hand. The same principle would apply to your flaccid length.
A manual stretch would likely exert more physical force than your body's ability to pump blood into it. That's not to say blood flow is weak, but I'm guessing that biceps are stronger.
Finally, and perhaps most fundamentally, an erection fills the penis in all directions / volume ... length and girth (height / width). But when you do a flaccid stretch you are only stretching in one direction ... length. So you don't have the other competing forces at play that a full erection has.
