Best to wear it as long as it lets you. Before your glans turn purple or cold etc. Wear it as long as you can safely.

Usually in an 8 hour work day I can get in at least 6 hours. Like Steve said go as long as you can without problems then take a short break and resume.

I notice that after an hour or so, I am able to move the bars out a bit more to maintain the same 800 gr tension. It is almost like the tunica and/or ligs have reached a point where they can no longer resist as much and need to give up a smidge more. I can sometimes move the bars out an additional 1/8 inch, especially if I reheat my rice sock. However, my max time is 1.5 - 2 hours per session.

would a 1 hour break be too much?

IMHO it really doesn't matter, just listen to your body. Normally, I can wear the extender for the first "session" for about an hour and afterwards take it off every 30 minutes.
But it really depends on what system you use (Noose or Strap) and at what tension you extend.
If you extend at a low tension, you will be able to handle longer sessions.
My tension bars have no indicator for tension, but for the last month or so, I've been using the extender with the bars almost completely compressed, so thats why I only last 30 minutes or so.
