Same here. My guess is that blood accumulates there either on the way to firm up your tip or from firming up the tip. Kind of like a blood log-jam. Then once it clears out, concurs gravity or whatever you have your good old boner.
As far as your dick getting thinner when completely hard, don't worry about it. The softer fatter one will deform more under pressure and not seem as strong, where-as the other one will feel like a lead pipe.
