I would like to announce that I am no longer a member of the pencil dick club. I am consistently over 5" in girth after my off days and even hit 5 3/8" with expansion one day! All I can say was HOLY HELL was that awesome to hold in my hand. I started over 2 months ago at a barely(and I mean barely) 4 3/4" girth. I measured with a semi and a light ui to get an extremely consistent measurement. I do all of my official measuring the morning before I begin my next set of workouts after my rest day.
I have been using a manual routine that looks like this currently though it started off much easier. I used a basic progression to get to this point, and yes I know that I haven't been PEing for that long but I took it slow and really watched what I was doing. No injuries to speak of..

Depending on my schedule and PIs.
Warm Up- Electric Heating pad 10-30 minutes(I watch tv, check my email, read the forum, etc).
Manual stretching during the warm-up. Maybe 2-3 minutes of tension continuing to warm in between sets.
100(50 if you count both hands as one, I don't) Reverse OK Grip Jelqs.
1 10-15s Reverse OK Grip Squeeze(Moderate Intensity)
100 Reverse OK Jelqs
2 10-15s Reverse OK Grip Squeeze(Moderate Intensity)
100 Reverse OK Jelqs
3 10-15s Reverse OK Grip Squeeze(Moderate to High Intensity)
100 Reverse OK Jelqs

Warm down in the shower with some manual massage and light sqeezing.
During off time I light squeeze a semi to expansion ever couple of hours when I go to the bathroom to try and keep my expansion. I also use my heating pad and do some light sqeezing, massaging, and some stretching during my off times as it seems to only positively affect my recover time.
I hope my experience can be useful to those other guys who suffer from having a pencil dick.
