I incorporate 4-6 hours of extending at heavy tension as I feel the lighter weights just don't give enough stretch plus I can't wear it in the evenings so 4-6 hours is all I can do.

It's working for me though I also hang 5lb ish for 15-20min at the start and then again every 2 hours.
      Pre PE - 7.75" BPEL - 7" NBPEL

      Current - 8.3" BPEL - 7.7" NBPEL

Not an extender user, but wouldn't more time under tension create more gains? Since you are dealing with ligaments that take a while to heal and adapt to the stress placed upon it, less intensity and a longer duration will allow it to heal over time and stretch the tendons out without it being too intense and too much volume to cause you to under recover/over train. The only thing that will allow you to heal faster is bloodflow, zinc, vitamin A and C. I would say protein as well but I don't believe a study has been done to see if that helps your shaft heal. It does help with wounds though.

I'm assuming more time in the extender is better than more intensity for a shorter duration. That's just a theory. Of course the weight you use is subjective because other might need more or less weight depending on what works for them.
