  • My quest to find a true premature ejaculation treatment has just started

    Hello everybody! I’m new in the forum and I would like to present myself. I’m from Lisbon, Portugal, I’m in my 40s and I suffer from premature ejaculation (PE) since I can remember, which means since I was a teenager. I never had sex so far, not just because of my PE problem but also because I suffer from extreme shyness, social anxiety and even panic attacks and agoraphobia in the past. I’ve never tried a PE treatment of any kind because I’ve always thought that the premature ejaculation thing was due to my shyness and anxiety problem and since this was sufficiently serious to keep me from having a normal life and socialize I’ve always let things run on their own and kind of accepted this condition (like my chronic anxiety) as something untreatable.

    It was only when my psychological problems got worse (to the point of having to be hospitalized in a psychiatric clinic for two days) and have been compelled to take an anxiolytic and an antidepressant) that I accidentally discovered that the medication had the side effect of delaying ejaculation, which later I confirmed by doing some research on the internet.

    Now that I know firsthand (along with some research I did on the internet, especially after reading some serious research papers in the sexual medicine field) that premature ejaculation is a neurobiological condition, regardless of one’s willpower or commitment to psycho-babble techniques like the stop-and-go, the squeeze technique and other similar cognitive-behaviorist approaches, which only work (at the best) with men who did not have previously the condition and develop it at a certain point in their lives; and it is not a result of my anxiety or my shyness or my lack of experience, but something correlated with these and probably with the same genetic roots, I have paradoxically gained hope that some drug (present or future) can or will handle this problem safely and efficiently, until a permanent fix is available, most certainly through gene therapy and/or nanotechnology applied to the biomedical sciences. So I started searching the internet for experiences that others might have already made and that have resulted well enough with them. That’s how I found this forum.

    As I said I don’t believe in any treatment that appeals to one’s willpower, commitment or conscious effort (like the stop and go or the squeeze techniques above mentioned) as I think this is not a matter of training, concentration or willpower but something neurobiological as are the anxiety problems that I suffer from. You may be thinking: «aren’t you already taking an antidepressant»? Yeah, sure, but it only took effect in the first two years or so (I’m now in the seventh year of daily clomipramine). Over time the “delaying effect” slowly decreased until I barely reach five minutes (or less) nowadays – and I’m just talking about masturbation alone! I’m sure that in the event of sexual intercourse would be even worse! As I said I never had sex so far as my fear of failure (or feeling humiliated before a woman) and my anxiety makes me further postpone my début on this domain. Nevertheless, I’m planning to search this forum for your personal experiences (or should I say, «experiments») as well as share my own in the next months. I’m presently taking 500 mg of L-arginine daily, together with the 50 mg of clomipramine that I’ve been taking in the last seven years, hopping to reach an “augmentation effect”, but I haven’t seen any increase in the time to ejaculation. The next step will be doubling the daily dose of L-arginine after finishing the first bottle of capsules.

    Hey Daniel, welcome to the forum.
    Well, as you wrote, your situation is not the easiest, but you made the biggest step ever to make the change. You've acknowledged the problem.
    I would concentrate on your social anxiety first, as it most likely seriously limits your life and overall happiness.

      Hi sexmaster! Thank you for the welcome message.

      As you may already noticed from my initial post, I don't believe primary (lifelong) premature ejaculation (PE) can be treated with cognitive-behaviorist approaches (like the stop-and-go, or the squeeze or similar techniques) but with the right synthetic drug (SSRIs are the first evidence-based sexual medicine approach to PE, though not the perfect answer yet) or the right combination of natural remedies taken regularly, on a daily basis.

      From my readings, it seems the serotoninergic system plays a great rol in PE. This accidental finding owes much to the sexual side effects men reported while on these antidepressants, so it is at least conceivable that natural substances that target the same serotoninergic system may prove to be a great help to tackle the PE problem, like 5-HTP or SJW, just to name a few.

      I'm currently on 50 mg clomipramine (an older kind of antidepressant from the tricyclic class) and 500 mg L-arginine daily and just finishing the first bottle. I haven't seen any difference from taking clomipramine alone so maybe the 500 mg L-arginine dose is too low. Next weekend I'm planning to double it for 1000 mg a day. If it still doesn't make a difference I think I will try fenugreek, L-thianine or valerian.

      I'm planning to post a list of substances (natural remedies) used in phytomedicine which I found often cited/recommended while surfing the internet and wait for other forum members to give their input.

      I whish you the best luck for your efforts to solve your PE and ED problems too, although your approach is somehow diferente from mine and centered on physical exercises.

        Hi daniel, Welcome.
        You've got a good understanding of the neuro side of things and obviously your anxiety has contributed to your pre e but, and I don't want to sound like a dick, you are wrong about the physical side and that no will power and conscious effort have an affect. It's that complete opposite and unfortunately your anxiety has been so chronic that it has led you to believe this, of course it would having such a huge impact on your life.
        Success with the meds is also not only down to the psycological, neurobiological but physical aswel ( in my opinion more so ) as antidepressants, ssri's snri's etc. affect the body aswel and with the one I was on for example ( tramadol ) it was the sedative affects ( coupled with the increase serotonin ) that was impacting the delayed ejaculation.
        You've not lasted long during masturbation throughout out your life so of course without practicing, training to be able to prolong it ( without meds or anything else ) you never will.
        Years and years of quick masturbation and never the opposite ( be it sex or solo ) has meant you have developed pre e, and only doing the opposite will reverse it, along with relieving your anxiety, and adressing any possible other physical issues ( pelvic floor, general fitness/ health).
        You will beat pre e and although your anxiety doesn't help it, if you follow the advice of the members on this forum, you will beat it, chronic anxiety or not, honestly. But it will take will power, it will take conscious effort, it will take time amd dedication. But if you ask yourself would I rather rely on meds / supplements, or something else for the rest of my life or woukd I rather tackle this and be able to beat it without any of those things.
        Don't succumb to your issues, use them to drive you and you motivate you to do what it takes to achieve your goals.

          I honestly think that in order to exchange our point of views about PE we must first disclose our fundamental view of the world and of the human experience and the way we humans experience the world around us. That means I must first understand where do you stand in the so called «body-mind problem» - please don't take it as a demonstration of snobism or intelectual arrogance from my side.

          I currently think our mind - that includes our feelings, thoughts, emotions - and the resulting behaviours are deeply influenced by our neuroanatomy and neurochemistry, and every feeling, thought, emotion has a neurobiological correlate or substrate. That's why I believe Primary PE (please bare in mind that tere are at least 4 classified types of PE according to the ISSM) must be addressed by the same tools which led to its onset - in the case of Primary PE, since it's present from early puberty onwards, a genetic cause is suspected the best candidate for its explanation (contrary to something which was acquired later in life), and when we say genes we say the biological products they code for - be it a protein, an enzyme, a hormone, a neurotransmiter, etc.

          By neurobiological I also mean physical in the sense that most certainly every feeling, thought, emotion, memory which is generated in our brain has a physical susbtrate - a protein, an enzyme, a hormone, a neurotransmiter, etc. In this sense I think we agree. My doubt (which seems to me you don't have) is if exercising or taming a given behaviour - like masturbation or sexual intercourse, whether through edging, stop-and-go or squeezing the penile gland by a sexual partner - is sufficient to change the underlying neural network and neurochemistry involved in it. I hope this line of thought may elucidate what I mean by neurobiological and why I find it very unlikely to treat PE by means of edging or any other physical approach like breathing exercises, yoga or meditation. The excitement of the sexual act is so overwhelming that levels down any concious methodical effort you may put in it. That's why so many members of this forum seem to strugle to increase substantially their intravaginal ejaculatory latency time (IELT) and conquer their PE problem for good.

            I like that you have looked so deeply into this, clarifies my belief that you are willing to do anything to beat this and thats the most important thing.
            You said primary pre e is suspected to be genetically caused. In the early years ( from 12 up ) sex has not occurred and masturbation is only a new discovery. Most guys ( don't know statistics but I'm betting almost everyone) will ejaculate quickly, some will learn that by prolonging the act they get greater pleasure and stronger orgasm. Unfortunately ( for us ) we think " masturbation= ejaculation=pleasure" and the quick trigger starts to develop. When the time comes to have sex with a hot girl it's over as quick, and we scratch our heads and say " what's wrong with me". We look at porn and wonder in amazement how they can last so long. We wonder why friends can brag about 15-30 min sessions when we're stuck at 1 min.
            We look up causes, possible causes and possible solutions. We experiment with different things we look for and unintentionally stumble on and think there working for different reasons, desensitising, relieving anxiety, increasing serotonin or dopamine.
            When we attempt to replicate it sobre and fail, we assure ourselves of those recent " findings"
            " yeah definitely anxiety, 100% my low seretonin levels are the cause ". But without attempting to beat it ourselves with no help, we never realise that these issues are the same issues that we face in almost every aspect of life and yet they do not engulf us and what we do.
            ( in your case your anxiety is severe and obviously has made it difficult for you, but I am sure you can overcome it one way or another).
            Genetically, look at Lionel Messi, best footballer in the world, told he could never play football because of his growth disorder ( minor form of dwarfism I think). Yes he had growth hormone treatments to help, it wasn't pure grit and determination that allowed him to grow a foot and a half. But you know what I'm saying, we can't surpass the limitations we set ourselves, if we submit to our own impairments, inadequacies and shortcomings we won't be able to excel.
            Pre e is mind and body, your mind has to be in the right place before the body can start adapting.
            Is the l-argine and cital working for your anxiety?

              The evidence is in this forum that pre e can be and is cured by training . Guys always want a pill, rather than hard work and discipline .

                    It's widely evident. Read around, you will be convinced if you see the progress people have made. We wouldn't push it otherwise, and would advise the easier options, pills, delay sprays etc.
                    I genuinely want you to beat this because I hate it and what it does to a person. If I didn't think you could I wouldn't waste my time.

                      Do you think that 30 years of it is not enough? Do you think that male porn actors need any effort to stand for 15 minutes or so? Do you think that they kegel or breathe slowly or think of anything disturbing to put the break on their pleasure/enthusiasm?                       Taking longerlastingnoob's words and wraping them with my own conclusions: everyone of us is a croossroad, a delicate intertwine between genes and environment. No one is born already mastering football. Even Messi had to practice and evolve. But I'm sure an honest contender of my point of view must aknowledge that if he hadn't the "gift", the potencial, the drive, the predisposition to be the player he is not even the most solicitous of the nerds/geeks could be a Messi or a Cristiano Ronaldo. Taking what you're saying to the ultimate consequences would mean that there isn't anything (literally) you couldn't do if you put your time and effort to it - including flaping your arms and flying on your own, without any technical assistance.

                        longerlastingnoob, I'm new in this forum so please spear me the time of search in it and post a couple of links to a couple of exemples of members that had primary (i. e. lifelong, not aquired after a normal period) premature ejaculation and went from 1-2 minutes IELT to 10 minutes or so (the minimum I can classify as a normal/satisfying intercourse time lapse) so I can dig into it. It's not asking too much I think...

                          Your of the belief that we are limited by what our potential sets us. Messi was just one an example, it's everywhere, feats monumentally more amazing than beating pre e. 30 years of self doubt ( not that I don't think it hasn't been warranted ) seems to have skewed your desire to achieve and be great on your own, which you already may be, I can't say for sure since I don't know you. Just my assumption, correct me if I'm wrong.
                          Wether or not you believe that porn stars are naturally that good or just normal guys using everything at their disposal to help, it doesn't matter. The evidence is here, and it is possible to not just beat pre e but have full control of yourself.
