When it comes to lifelong or primary premature ejaculation, it is most often accompanied by an underlying cause such as hormonal, neurotransmitter, vitamins or cholesterol imbalance. So as a first step, one should get tested to any underlying cause. Once this is fixed, premature ejaculation gets fixed itself. Normally premature ejaculation is accompanied with many other ailments like anxiety, depression, insomnia, hyperactive brain, hyper-sexuality, week muscles, uncontrolled muscle spasms or convulsions.

Neurotransmitter Imbalance
NEUROTRANSMITTERS are the brain chemicals that communicate information throughout our brain and body. They relay signals between nerve cells, called “neurons.” The brain uses neurotransmitters to tell your heart to beat, your lungs to breathe, and your stomach to digest. They can also affect mood, sleep, concentration, weight, and can cause adverse symptoms when they are out of balance.
Most importantly, this imbalance causes premature ejaculation, nerve disorders, hypersensitivity, anxiety, depression and other mental illnesses. Mental illness is under diagnosed through out the world. A study showed that 42% lifelong premature ejaculators has ADHD.
Neurotransmitters control your arousal, erection and orgasm etc. Each one of us has different levels of these neurotransmitters and this determines our general behavior and personal traits.
There are excitory neurotransmitters like histamine, norepinephrine and dopamine. High levels of these cause over excitement, over arousal, over sensitivity and over activity.
There are inhibitory transmitters like serotonin and GABA. These calm your body, brain, nerves and muscles.
Balance between excitory and inhibitory neurotransmitters is very important. If the balance is in favor of excitory transmitters, then you are over excited, over thinking, anxious, ambitious, hyper-sexuals and premature ejaculator etc. You fall in Category-A personality. They have less control over their body, brain, muscles and nerves etc. They have hyper active brain.
If the balance is in favor of inhibitory transmitters, then you can enjoy good sleep, sex etc. In fact, this is what makes your personality Category-B. They have better control over their body, brain and actions.
Normally, the ultimate balance of neurotransmitters is determined by the body metabolism through a complex feedback mechanism. When you increase one neurotransmitter, your body tries to achieve the balance by reducing or enhancing other chemicals, hormones or neurotransmitters. That is why it is normally not advised to play with these chemicals unless you have some serious illness or disorder.
Neurotransmitters for Sex:
A. Serotonin, its deficiency causes premature ejaculation, anxiety and depression etc. That is why, the first thing a doctor prescribes is an SSRI drug (though it is an off-label use).
Recreational drugs and weeds including opiates etc., also increase the serotonin.
Exercising and working out muscles increase the serotonin and should be the first choice to increase serotonin.
Supplements: Griffonia Seed Extract or 5-HTP, St. Johns Wort, L-Tryptophan, Ashwagandha, Vitamin B-Complex.
B. GABA is the second most important neurotransmitter. Lack of it makes the nerves hyperactive and causes premature ejaculation, insomnia, restless legs syndrome, bipolar disorder, nerve control and motor control disorders etc. Severe deficiency of it causes epilepsy. If drinking alcohol makes you last long, increasing GABA will most probably help you last long. Alcohol increases GABA in the brain temporarily. Psychotic drugs like Valium also increase GABA. But these are addictive at the same time drugs.
Taking vinegar 1 teaspoon before every meal increases GABA. Valproate (valproic acid) is more better than vinegar as it does not get digested within minutes like vinegar. Lithium also works.
Supplements: Glutamine, Valerian, Green tea or L-Theanine, St. John's Wort, Vitamin B6, Magnesium, Passion Flower, Ashwagandha, Kava-Kava. Almonds, Walnuts are also good foods. Melatonin also increases GABA.
(You know there are sleep pills which increase the GABA but these are habit forming. Passion Flower or Passiflora in homeopathy is known as herbal valium. So it seems a good choice to increase GABA)
Tip: Lower your glutamate levels with Vit K2 and B6.
C. High levels of Histamine:
It is big culprit. It causes inflammation, allergies and makes the nerves, muscles, and prostate over sensitive. It causes premature ejaculation, anxiety, over stimulation, aggressiveness, compulsive and a racing brain, high libido, allergies, and low muscle tone. Easy orgasm and high libido is a major symptom of high histamine "histadelia". A saliva or blood test can be done to check your histamine levels. In homeopathy, the term "Sulfur Patient" is used. You can google these terms and find out more detailed symptoms. I think high levels of histamines is a big factor in premature ejaculation (if not the prime factor).
According to "The Amino Revolution" book, premature ejaculation in younger males is due to high levels of histamine.
Dr. Pfeiffer mentioned premature ejaculation as a symptom of histadelia in his book (page 155). This topic is worth reading. On page 160, he prescribes following to lower the histamine:
In summary, the treatment program for histadelia is:
1. Vitamin C 1.0 gm AM & PM
2. Vitamin B-6 to normal dream recall
3. DL-Methionine 500 to 1000 mg AM & PM
4. Calcium salt 500 mg AM & PM
5. Phenytoin (Dilantin) 100 mg AM & PM
6. Low protein diet
7. Avoid multivitamins and folic acid

Here is a list of histamine lowering foods. Some of these are often found in premature ejaculation supplements.
Sulfur rich foods (like ginger, garlic, onions, turmeric, eggs, cauliflower, broccoli) are good to reduce histamine. Quercetin, Nyquil, butterbur, Thyme also lowers histamine.
Remember these foods are very popular among some communities.
Most effective supplements are MSM, SAMe, Methionine, L-Arginine/Citrulline, Calcium, Vitamin-C, inositol, TMG, and zinc.
Avoid: Folic Acid, Alcohol, Histadine, Sugar
Delayed ejaculators are given histadine to increase histamine levels and cure DE.
D. Dopamine is a pleasure and motivation neurotransmitter.
High levels cause over excitement and over arousal and therefore cause premature ejaculation. If your refractory period is low and you have throbbing erections, then you are high in dopamine.
If you have erection problems or ED, then you are surely not high in dopamine.
Anxiety, psychosis, addiction, compulsion, gambling, aggression and unhealthy risk taking are some symptoms associated with increased levels of dopamine.

Low levels of dopamine cause Parkinson's disease etc. In this case Macuna Prurens is used to increase dopamine. I have seen Macuna Prurens as part of herbal medicines for premature ejaculation. If your premature ejaculation is combined with ED, then increasing the dopamine is worth trying.
E. Norepinephrine is known as the arousal and orgasm neurotransmitter. It is synthesized from dopamine. It causes stress, anxiety and hypertension. Make sure your stress and anxiety is not causing it to increase. Have good sleep and exercise. Use anxiety herbs like passion flower, Valerian etc.
Note: If you also have insomnia, then most probably your norepinephrine is high.
F. Nitric Oxide is also very important to relax the smooth muscles and calm the nerves. If you have erection problems or erectile dysfunction associated with premature ejaculation, then you are low in nitric oxide. That is whey nitric oxide boosting herbs are also part of premature ejaculation food supplements.
In today's advanced world, L-Arginine/Citrulline is a miracle for boosting nitric oxide. It is widely used supplement in the muscle building community because high levels of nitric oxide are required for building and healing muscles.
Ashwagandha, pomegranate, watermelon is good for increasing nitric oxide production.
Mineral Imbalance
Mineral Imbalance is another important factor in premature ejaculation. It greatly affects the neurotransmitter balance.
Zinc, Magnesium, Potassium, Selenium, Copper, Manganese, Calcium etc., are also very important.

Summary:
If you are making slow progress with pelvic floor exercises and edging etc, there is no harm in trying some supplements. If you have life long premature ejaculation, you must consider lowering your histamine levels. It is what causes spasms in the pelvic floor area and makes the mental arousal uncontrollable.

Chemical Aspects of Premature Ejaculation

This is my kind of stuff!!! The biological / biochemical approach is the only one that has some potential as a fix for what concerns lifelong (AKA primary) premature ejaculation. All other stuff I read in the PE Forum section is pure rubbish!!! Sorry if I'm so direct but I'm tired, alone and frustrated for so many years I don't have the patience to be polite anymore.
You see, my only problem with the list of substances you present is what to choose from. For instance: in what regards A. Serotonin the supplements you mention (Griffonia Seed Extract or 5-HTP, St. Johns Wort, L-Tryptophan, Ashwagandha, Vitamin B-Complex) are to be taken all at the same time or should they be tried one by one? The same goes for B. GABA: Glutamine, Valerian, Green tea or L-Theanine, St. John's Wort, Vitamin B6, Magnesium, Passion Flower, Ashwagandha, Kava-Kava. Almonds, Walnuts and melatonin are to be taken together or one by one until you find one that hits the spot?
I'm going to be more personal now. Since my early adolescence I've suffered from extreme anxiety, panic attacks and agoraphobia. I also suffer from mild depression and mild OCD (which is also an anxiety disorder) but I think my mild depression comes from my low self-esteem, lack of confidence, shame and inferiority complex. Nevertheless I can be wrong and both (anxiety and depression) might be two conditions with the same underlying cause. Anyway I think depression comes from chronic anxiety/fear (social phobia is a big issue too by the way); and sleeping problems too: either I lack the need to sleep at night, or my sleep is not restorative (too superficial?) or I wakeup too soon.
With this complex mix I always find hard too know what exactly should I aim first: anxiety or depression? If you ask my personal opinion I think the main problem is anxiety; but benzodiazepines are addictive and the body adapts to it in time, demanding for dose escalation to achieve the previous effects (plus I never found any research article on the usage of BDZs in PE). I've been taking Anafranil (Clomipramine) for 8+ years now and I think all the positive effects it had in the beginning are all almost gone (delayed ejaculation for sure, it only lasted 1 or 2 years I think). The problem is my anxiety was so high prior of having started with clomipramine that my palm hands were sweat, my heart beat heavily and I often had the urge to go to the bathroom (this became a problem on its own that lead me to stay home). In a way Anafranil was a blessing because panic attacks and agoraphobia went away, I managed to have my driver's licence and visit a few cities on my own, things I thought I could never do before taking Anafranil. But as I said lately I've been feeling more anxious, I have trouble sleeping, I feel tired and sad... and my PE remains the same as usual. I continue to take my 75 mg of clomipramine and occasionally I also take 450 mg of valerian root extract at night or 1 mg of melatonin at night or 1 capsule of passiflora extract at night, it seems with no positive effects.
Having told you my story and my symptoms what would you recommend me? I would appreciate very much your guidance on this (that is treating anxiety and PE through biochemistry). Thank you for your time and patience.
