I would just like to add that while there's some truth in what Unita says re:don't try to do a whole life makeover, I think that is actually possible, and sometimes more effective than chipping away at onesself. It's about setting yourself a goal and sticking to it. The difficulty is that bigger goals are harder to stay committed to because they take more work, and more strength & resilience against falling short. Only you know how committed you'll be to your goals, and setting realistic goals is a totally relative measure.
At the end of the day, it's up to your commitment, nothing else, and I'm right behind you man.
