Research "vaginal tenting"...once aroused, females can accommodate VERY large penis lengths without pain, usually; however girth can be an issue, depending upon her vaginal entrance diameter. It's kinda like a "deep throat" in oral sex, the gag reflex and ability to swallow can be trained to accommodate, the diameter of the mouth is the limiting factor.

I doubt that any tenting is going on in a professional setting. Its usually 2min filming & fucking, 2min pause, ... With various positions and such.
