Vaginas can push out babies with a lot of hormonal changes and the body dialating the cervix and preparing the whole shebang for childbirth. A penis can absolutely cause damage if it is too big and can certainly cause a shitload of pain as well, regardless of if she is warmed up or not. The hormones that prepare a woman's bodies for sex are very different than the ones that prepare her body for childbirth.

The answer is quite simple - porn is not a representation of true life.

So, pain in sex aside, and mind numbing drug use aside (I think that is actually a greater issue than anyone realizes), I suspect that the porn producers probably intentionally pair up men and women who might be more compatible in that area.
