          WHAT EVIDENCE IS THERE THAT SHOCKWAVE ISN'T JUST ANOTHER SCAM?

          How many times have you seen products that are supposed to regrow hair, or turn your dick into a staff of steel? Before I bought this machine, and decided to use it I wanted to make sure it wasn't just another modern snake oil cure designed to take advantage of our aging bodies and full wallets. Testimonials don't do much to prove anything and you can find plenty of those online. Gainswave appears to be the largest promoter of shockwave therapy, and they have lots of scientific studies that show that it works. I give them below so that you can decide for yourself if there are actually teeth to the claims.

            SECOND SIGN THAT IT'S WORKING

            It's been about 6 treatments so far. This is usually all most commercial treatments do, but the studies show the results take a month or two to kick in.



            Generally I take 100mg Viagra and a small dose of trimix. This usually this works OK, but the effect starts to fade toward the end and sometimes I end up having to put a ring on to finish. The difference today was that I stayed hard throughout, and even maintained an erection for a while after.


            I first just took Viagra to see what would happen. Now, Viagra by itself hasn't worked for me for years. However, I was actually able to penetrate for a while with Viagra alone. That was an improvement, but it wasn't hard enough to sustain so I had to give myself a trimix injection. The nice thing was that I wasn't worrying about my erection, but was able to focus on my wife instead. It feels like progress to me.

              Could you describe the sensation during and after the treatment ?
              I 've just done my first treatment yesterday, it didn't hurt that much if felt like a piezoelectric arc is shocking it repeatedly, the doc said that is entirely correct.
              My dick felt really shocked and tired after the treatment, I feel worse today, really numb in that area and my libido dropped massively .

                It doesn't hurt, and my skin doesn't get red. When I do the crura sometimes it feels like a small pinch or shock. I haven't noticed anything negative in my dick afterwards, but in the literature I've read they mention that some people feel that way. Remember that what makes it work is that you are causing trauma which causes your body to reaction and start to heal the trauma. Can you find out what settings are being used? I'm doing 15Hz and I've read about 200Hz treatments. Remember that it takes time to work. What seems unusual to me is the numbness the day after.

                  UPDATE ON RESULTS

                  I mentioned that my cock is now longer when not erect. I use a pump once a day to inflate and deflate my cock about 5 times. When I started I had to pump a lot more before it got completely erect. Now it gets there much quicker. I can only imagine that this is due to better blood flow. Now my goal isn't increasing size, but when I started, the vacuum device would get me to about 5.5 inches (14cm), and now I max out at 6 (15cm).

                    I'm roughly aware how neovascularization occurs and I do feel things happening mostly around my pelvic floor and I didn't get any unstimulated erections since then, but the semi boners I got were way different, like a feeling I'm more connected to my penis. I hope that the numbness is going to stop and to get a good rebound . I'm going to ask the frequency and the machine next I'm at the doc, I'm aware it can take days, weeks, or months to see results .

                      RESULTS AFTER 8 TREATMENTS

                      I've done treatments every other day for two weeks. Yesterday (Aug. 28) was my eighth. The good news is that I've had morning wood three days in a row. The bad news is that last night I took my usual dosage of trimix and had the usual problem keeping it up long enough. It's important to remember that in the medical studies that compared treatment and non-treatment groups, there was no difference until a month or two had past. In other words, it takes a while for the new vessels to grow. I've only been at it for two weeks. Gainswave does a few treatments, then waits a few weeks and then administers a few more, so they realize that it takes time as well. I also noticed that in many of the medical studies they did not accept patients who had experienced ED for a long period of time. In my case it has been about 17 years which is another reason I can't compare myself directly to those studies.

                        QUESTIONS ABOUT SHOCKWAVE FOR ED
                        The more I study shockwave therapy to treat ED the more questions I have. It is obvious that it works for a lot of people, but I understand why it hasn't been approved by the FDA in the US yet. I'm sure they ask themselves a lot of these same questions.
                        • What is the optimal shockwave strength?

                        Most studies take .09 mJ/mm2 from one of the first studies and use it, but I've seen others that use .12, and .25. Is stronger better or too much?
                        • Where should the treatments be applied?

                        Some studies apply the shocks to the top, middle, and base of the penis, plus the crura. Others apply shocks along the whole length of the penis. Some also apply to the top. Others only apply to one side supposing that the wave carries to the other side. Does it matter?
                        • How many shockwaves per area treated?

                        This ranges from 600 to 2000 and more in the studies I've seen. Does applying more get better results? Is there a point of diminishing returns? Can too many do damage rather than help?
                        • How often should treatments be done?

                        I've seen once to three times a week. Some apply lots of shockwaves in one blow, and others spread them out over a few days.What works best?
                        • How many total treatments?

                        This typically ranges from 6-12. I think the low number reflect the fact that researchers want to get quick results rather than do a long study. In a clinical setting people probably can't afford 20 treatments. Maybe more treatements give better results or more long lasting ones. Maybe people with more severe ED benefit from more treatments.
                        • Is it helpful to take a break between treatments?

                        Gainswave does 3 weeks of treatments, a three week break without, then another 3 weeks of treatments. Would 9 weeks of treatments without a break be better?
                        • How many shockwaves per minute?

                        Some studies apply at 120 or 200Hz and others at 5. Does it matter? My guess is that it doesn't matter how quickly you get them, but how many.
                        In a way it is heartening to see that in spite of the many different protocols used, most produce positive results. It will take a lot of experimentation before we know what the optimal treatment parameters are.
                        • What are the success rates?

                        Gainswave claims 80% if I remember correctly. Different studies report highly variable success rates from about 30%-80% Would you lay down $4000 for a 50% chance it will work?
                        • How long does it take to work?

                        Some anecdotes report results after one treatment. If plaque build up is the problem I imagine that may be possible. Most studies indicate results take a month or two. I'm sure it's different for each person.
                        • How long does it last?

                        Here's another one that varies a great deal. For some people it lasts a month, or three, or a whole year. It would be nice to know if repeated treatment boosts the effectiveness after it starts to wane.

                          STRONGER TREATMENT GIVES POSITIVE SIGNS

                          In the medical studies some people use stronger shockwaves, and apply more of them that I have been. Others who have had the treatment report that they come away with a cock that feels like it's been through something. Since I haven't felt that way I decided to up the strength from .09mJ/mm2 to .12 in the 9th treatment. (That's 120mJ on the machine setting.) I also doubled the number of pulses to 2000 on each side and 1000 on each crus.



                          I applied the treatment in the morning and afterward really didn't notice any difference, so in the evening I tried an experiment. Two days before I had used a 7ml dose of trimix which didn't last very long. That had been my typical experience with that dosage for the last few months; it just wasn't lasting as long as it used to, and as long as I needed it to. So, at night I gave myself 2.5ml to see what would happen. Wow, that smaller dosage gave me a good boner (#3 to occasional #4 on the EHS) for about 90 minutes.


                          The question I have is whether this difference is due to my stronger treatment or if it was coincidence. The larger dosage hadn't worked two days earlier, but I had given it to myself after using the ring. Would that change anything? I really don't know. Could I have botched the injections the last few times. What I mean is that sometimes I've not put all of the medicine into the corpus cavernosa where it belongs. I kind of find it hard to imagine that so many of my injections were poorly placed, and that the 2.5ml one just happened to hit the right spot. Time will tell.

                            RESULTS AFTER 12 TREATMENTS

                            Maybe it's overkill to report results so early since most studies show they really don't show up for a month or two. Anyway, in the last few treatments I upped the energy flux density to .38 mJ/mm2 which is 180 mJ on the machine. It really doesn't feel any stronger.


                            So, the negative news is that during oral sex, my erection stayed at about a 2 (on a scale of 1-4), which is how it has been for many years. The positive news is that as an experiment I gave myself a 1.5ml injection of trimix. Now, before I started shockwave treatments a 7ml dose would only keep it up for a few minutes, not long enough to finish. But now, this small dose kept working for over an hour. Other positive signs are more morning wood.

                              Came across your thread. Great to find another fella DIYing a shockwave machine after reading the many studies online. The prices clinics are charging are downright robbery.

                              Purchased a SW8 from 1688/alibaba and should be here next week. Paid about 1000 RMB. Also thinking of upping the frequency and values quoted in the studies. Either that or stick to it or lower but on a daily basis.

                              Anyway, I'm pretty sure the goal for this therapy is angiogenesis and increasing blood inflow, while venous leaks are more about excessive outflow. Either way I can see this being beneficial to penile health in preventing negative smooth muscle atrophy and changes in individuals with poor erections. I'm thinking of combining it with VED and other angiogenic substances like topical minoxidil.

                              FWIW I've seen great improvement from physical therapy (seeing a PRI certified therapist) in regards to my moderate venous leak. But I do have documented pelvic floor problems (wee bit of rectal prolapse) so that is likely why.

                                It's good to see you doing the same thing. As far as power is concerned, I do applications every two days. I started at .09 and did 3000 pulses. Then I upped it to .38 (180 mJ) and did 6000. I've lowered the power now back to .09, but I still do 3000 pulses. Let me know how it works for you.
