Creatine is not the only supplement that works. Many work but they are now illegal or banned by sports associations. But none of the supplements work if you don't bust your ass. They allow you to work harder and longer, and recover faster, but you have to work. As fas as side effects, the only ones I know of are water retention, bloat and diarrhea.

Sperm - Water retention and possibly a little bloating. Massive (20grams +) can give you the shits and some GI distress. Otherwise, it has no sides and will help you massively in the gym. Some of the newer creatines complexes (versus creatine monohydrate) have less bloat and GI risk. I am not going to say creatine added to my flaccid size on its own, but I found that my flaccid is hanging better over the last week or 2 since I started taking it.
It might be just water retention though from the creatine because that is what it does to muscles.Starting Point: September 2010: 6.5" x 5"

Creatine can only mess up your kidneys only if you don't drink lots of water or already have an existing kidney problem..LOG
